Role Overview
The Community Health Worker (CHW) acts as the bridge between patients, the community, and medical systems in order to remove barriers and increase wellness across all life domains. A CHW serves as a patient advocate and liaison, conducting proactive in-person and phone outreach to promote health literacy and increase access to resources.
