The Practice Transformation Specialist (PTS) serves as the primary interface between Aledade and partner practices. The role focuses on implementing ACO initiatives, supporting wellness visits, training staff and physicians on processes and technologies, and nurturing relationships to improve financial and quality performance in a value-based care environment.
